Our main clinic in Long Island City offers full-service primary medical, dental, mental/behavioral, and specialty healthcare, health education and life skills services for the entire family. We have a pharmacy on the premises and offer prescription payment options for uninsured and undocumented patients.

Appointment Checklist
We require the following when your client arrives at The Floating Hospital:
- Residency letter from shelter
- ID
- Children’s immunization records
- Insurance cards or other insurance information
- Social Security numbers for all being treated
- Healthcare forms
